REGISTRATION AND HOTEL RESERVATION INSTRUCTIONS
Registration
Fee:
The fee for attending this three-day conference is $150. The
registration fee partially covers the cost of lunches, reception, breaks and
conference materials. Your registration fee is non-refundable.

Hotel
Reservations:
You can make your hotel reservations
directly online or by calling the hotel. Reservations for the Hyatt Regency
Chicago can be made via phone at 1-888- 421-1442. When calling the hotel,
ask for the conference room block. To register online, go to:

The room rate is $168 (plus 15.4% state
and local taxes, fees and assessments) per night for a single or double
room. The hotel requires a one-night deposit to hold your room.
You must make your hotel reservations by
July 10, 2009. If you do not make your
reservations with the Hotel by this deadline, then you will need to find
your own alternative arrangements.
The hotel has a 24-hour cancellation
policy prior to the date of arrival. No-shows will be charged the full room
cost for the dates of the reservation.
