If you are interested in attending the 2008 Male Health Educator Training Institute, you must answer “yes” to the following:
If you are eligible and interested, please complete the Nomination Form. Send the completed forms to your RPC by the April 3rd, 2009 due date Upon receiving your Nomination Form the Title X Regional Program Consultant (RPC) in your region will review all nominations from his/her region and, within one week, submit a prioritized list of nominees to the Office of Family Planning Central Office. The OFP Central Office will make final decisions on attendance and notify all nominees of their decisions. To be considered for the 2008 Male Health Educator Training Institute, your Nomination Form must be submitted to your Regional Program Consultant by April 3, 2009.
