Training Outline:

Days 1 & 2 (Required for Young Adults and Clinical Care Staff)

    Topics to include:

  • Differences between adolescent and adult HIV care services
  • Adolescent development
  • Needs of youth as they transition:  body, mind and spirit
  • Managing change
  • Roles and responsibilities of young adult consumer and clinical team
  • Young adults as mentors
  • Models for transitioning
  • Creating a transition plan for the clinic
  • Implementation planning
  • Challenges of transitioning
  • Supervision and Boundaries
  • Preparing clinical care staff for transition

Days 3 & 4 (Required for Young Adults and optional for Clinical Care Staff)

    Topics to include:

  • Education and support
  • Mentoring, confidentiality and boundary setting
  • Working on a team: treatment planning, case conferencing, problem solving
  • Cultural competency
  • Communication skills
  • Self Management
  • Life Skills Preparation, including knowing when to seek medical care, and identifying and describing symptoms; making, canceling and rescheduling appointments; attending appointments; call for urgent visits; requesting prescription refills; negotiating multiple providers and sub-specialty visits; healthcare insurance; entitlements; and establishing a relationship with the case manager

 

Application Process:

Who is Eligible

Ryan White Funded Part D grantees who work with HIV+ adolescents or young adults will be given first preference. Part A, B, and C Ryan White funded grantees are eligible to apply and will be contacted if space permits.

Who Should Attend

Adolescent clinical care providers should select two consumers who have recently transitioned to adult services. The Adolescent agencies should also send at least one clinic staff member to the training program, as well as an Adult clinical care provider who works closely with the Adolescent clinical care provider.

Agency Commitment

To receive the free training and technical assistance, agencies that participate must commit to the following:

  • Provide funding for travel, accommodations and per diem for two patients and at least one clinical care staff person

  • Participate in all project-related technical assistance activities

  • Participate in all evaluation activities
