SNPs Resources for Case Managers
| CAI
is working with the New York State Department of Health
AIDS Institute and the New York City Department of
Health to help educate consumers and providers about
the HIV Special Needs Plans for Medicaid recipients
living with HIV/AIDS.
The
HIV Special Needs Plans are special health plans for
people on Medicaid living with HIV/AIDS and their
children up to age nineteen, whether or not they have
HIV or AIDS. The doctors, nurses, and other care providers
who participate in HIV SNPs understand the special
needs facing people living with HIV/AIDS. SNPs providers
understand that consumers may need help with:
- taking
medication
- alcohol
and substance abuse problems
- talking
to loved ones about HIV

All
HIV SNPs primary care providers are "HIV specialists."
This means that these providers have special training to
treat people with HIV/AIDS.
CAI
has provides training to consumers with the purpose of educating
them about the SNPs plans and developing the skills to make
health care choices that are right for them and their families.
CAI also educates providers on issues pertinent to SNPs
care delivery including confidentiality and sensitivity
to people living with HIV/AIDS. Technical assistance services
are available to community based organizations whose clients
may be eligible for HIV SNPs.
